They are the perfect complement to bring health, social, and housing services together to support better health outcome for whānau in Te Toka Tumai and Waitematā.
The programme supports a more equitable distribution of outcomes for whānau through education around how to live in a warmer, drier home, providing intervention items and empowering people to access services within their communities. This is a true partnership approach to delivering a model of care centered around our whānau and our people.
